Birth Certificates
The Town Clerk Office is open Monday through Friday, 9:00 am to 5:00 pm
Three ways to obtain your own or your child's birth certificate:
IN PERSON
You must have IDENTIFICATION: Driver's License, Passport or Dept of Motor Vehicle Non-Driver ID only. Fee is $10.00. We do not accept personal checks.
BY MAIL
Fill out an application and mail along with a copy of your Identification and a $10.00 money order (no personal checks) to Helen Kopke, Town Clerk, One Niskayuna Circle, Niskayuna, NY 12309. Please enclose a self addressed, stamped envelope.
BY PHONE
Call (518) 386-4511 with a credit or debit card. Total fee is $16.00. Birth Certificate will be sent same day via regular first class mail. Total fee for overnight Fed Ex is $32.00. There are additional charges for Saturday delivery, special overnight or priority mailings.
ONLINE
If you are unable to call or come in during office hours, you may place a credit card order at www.vitalchek.com/ at any time of day or night. You will be asked to FAX your driver's license with signature and date. Once the FAX is received, Birth
Certificates are sent the same day. Total fee for overnight Fed Ex is $32.00 (default setting). Total fee for regular first class mail is $16.00 (you must choose this on the drop down menu at the top of the billing page if you live locally or want to save on shipping charges).
