Birth Certificates
The Town Clerk Office is open Monday through Friday, 9:00 am to 5:00 pm
Three ways to obtain your own or your child's birth certificate:
IN PERSON
You must have IDENTIFICATION: Driver's License, Passport or Dept of Motor Vehicle Non-Driver ID only. Fee is $10.00. We do not accept personal checks.
BY MAIL
Fill out an application and mail along with a copy of your Identification and a $10.00 money order (no personal checks) to Helen Kopke, Town Clerk, One Niskayuna Circle, Niskayuna, NY 12309. Please enclose a self addressed, stamped envelope.
BY PHONE
Call (518) 386-4511 with a credit or debit card. Total fee is $16.00. Birth Certificate will be sent same day via regular first class mail. Total fee for overnight Fed Ex is $32.00. There are additional charges for Saturday delivery, special overnight or priority mailings.
ONLINE
For your convenience, you can process online requests using a credit card through an independent company with which we have partnered in order to provide this service (VitalChek Network, Inc.). The Town of Niskayuna Vital Records does not accept credit cards directly.
VitalChek can be reached through its website, www.vitalchek.com. An additional fee is charged by VitalChek for using this service and debit cards and all major credit cards are accepted, including American Express, Discover, Master Card or Visa.
You will be asked to fax your driver's license with signature and date. Once the fax is received, birth certificates will be sent within two days. Total fee for overnight Federal Express is $32. Total fee for regular mail is $16. You must choose either form of shipping from the drop down menu on the billing page.
