DOG LICENSE INFORMATION

TO COMPLETE LICENSE FORM:

Fill in Dog Breed, Colors (up to two colors or may list as tricolor), other ID or markings (if any) and Year of Birth of Dog.

Complete owner Information:  Name, Address, Phone

Note type of License with Fees:

State Fee:  For neutered or spayed dogs = $2.50
         Unneutered or unspayed dogs = $10.50 ($7.50 + $3.00 spay/neuter fee)

Local Fee:  $10.00 for all licenses UNLESS you are over 65 years of age,
          in which case the local fee is waived.

Add Above Applicable Fees for Total Fee:
           $12.50 ($2.50 for seniors) if spayed or neutered
          $20.50 ($10.50 for seniors) if not spayed or neutered


Make check payable to:       HELEN KOPKE, TOWN CLERK


Return license form and check along with proof from the Veterinarian showing that:
Dog is spayed or neutered
Rabies vaccine is up to date – proof needs to show date of vaccine

All proof will be returned to you along with the dog license tag. Note: The dog tag / license number is permanent. The only time a dog license tag is replaced is if the original is lost. A new tag will be ordered with your dog’s license number for a fee of $2.00.

According to New York State Law all dogs must be licensed.  In the Town of Niskayuna there is a Leash Law as well as a law that states you must pick up after your dogs.  See Police Department page to view Dog Ordinance.

All dogs residing in New York State by law must be licensed.
